Acceptable Photo IDs
Any photo ID issued by the United States government is acceptable as proof of identity, including photo IDs issued by Puerto Rico. Examples include driver's licenses (licencia de conductor) and voter ID cards (tarjeta de identificacion electoral), as well as others.
Acceptable Address Proofs
The following are acceptable documents ad address proofs for Puerto Rican customers.
- Housing leases
- Benefits award letters
- Water Bill
- Electric Bill
- Cable Bill
- Utility bills that do not include the zip code are acceptable if the rest of the address and the name are correct.
- Driver's License if the address is present on the card
Acceptable Subsidies
The standard eligibility programs that may be used as proof of subsidy include Temporary Assistance to needy Families (TANF), Supplemental Security Income (SSI), Food Stamps, Medicaid, National Free School Lunch Program, Federal Public Housing Assistance (FPHA), and Low-Income Home Energy Assistance Program (LIHEAP).
