Acceptable Subsidies
The standard eligibility programs that may be used as proof of subsidy include Supplemental Nutrition Assistance Program (SNAP)/Food Stamps, Medicaid, Supplemental Security Income (SSI), Federal Public Housing Assistance (FPHA), Veteran's Pension or Survivor's Pension benefit.

- This card is issued to participants of a non-Lifeline-eligible state-subsidized child care program to pay their child care provider.
Acceptable Subsidy Proof Examples
- New Card
- Both check and stub must be uploaded and the warrant numbers must match
- BOTH sides must be uploaded
Social Security Disability Income (SSDI)
- OHIO ONLY - Does not have to show an amount to prove eligibility
- Must be dated within the past 7 years.
- Must have "DI" at the end of the document number.
